More quality, less quantity

That could be the theme of a growing trend to encourage healthcare providers to focus more closely on an individual patient’s health.

Value-based pharmacy models call for the pharmacist to take a bigger role in a patient’s care. In addition to dispensing medication, the pharmacist will be part of the clinical care team.

This move is designed to improve health results and reduce costs. It calls for the pharmacy to offer patients early access to counseling and medication management classes, make assessments for patients with complex medical issues, and coordinate care with other healthcare providers.

Pharmacists also will step up efforts to encourage patients to take their medicine as prescribed. Poor “medication adherence,” as it is known, is a common frustration for pharmacists and physicians whose patients ignore prescription guidance. Two more are medication errors and ineffective management of medications.

Medicare and other agencies plan to move to value-based models by 2030. New systems will be established to reward better coordination across health care settings and improved health.

On the flip side, physicians may be docked for too many readmissions to hospitals and poor outcomes for patients.

The current fee-for-service system encourages quantity – more visits, more procedures, more tests.

The new model will emphasize quality – improving a patient’s care.
